Certificate of Liability Insurance and Worker's Comp Insurance, both must list DWM as the certificate holder. If you do not carry Worker's Comp. Insurance because your State does not require you to, you must sign the DWM exempt form, which will be provided upon request. If you are working in the State of NY, we require Worker's Comp. Insurance and cannot accept an exemption.

DWM must be listed as Certificate Holder on your Insurance as follows:
